As a Patient or Participant how can I access my file or medical records?

Enriched Health Care can provide individuals (patients/particpants) with access to their health information.

For access to your private health information held by Enriched Health Care you will need to ensure that your request be made out in the following way:

• be in writing
• include your name and address
• identify the health information being requested
• specify how you would like to access the health
  information (for example, if you want a copy of it or
  just to look at the information at their office)
• if you want someone else to access the health
  information on your behalf you will need to put the
  name of the person/organisation you are authorising
  to access your health information.
• Two forms of identification from the applicant (a photocopy of your driver's licence, passport, Medicare card, pension card, bank card etc.) must accompany the application

Enriched Health Care will charge a private health information release fee. This fee is payable prior to release of information. The current administration fee schedule can be found here.
